Privacy Inquiry & Video Access Request Procedure

Purpose:
This document outlines the procedure for handling individual requests for access to personal information collected through video surveillance, in accordance with British Columbia’s Personal Information Protection Act (PIPA).
1. Receiving a Request:
- Individuals may request access to their personal information captured by video.
- Requests must be submitted in writing to the Privacy Officer via email or mail.
2. Verifying Identity:
- The Privacy Officer must verify the identity of the requester before any information is disclosed.
- Acceptable verification includes government-issued photo ID and confirmation of visit details (e.g., date and time of visit).
3. Assessing the Request:
- Determine if the footage exists and whether the individual is reasonably identifiable.
- Evaluate if legal exceptions apply (e.g., third-party privacy, safety concerns).
- If other identifiable individuals are visible, footage may require redaction or restricted access.
4. Responding to the Request:
- The Privacy Officer will respond within 30 calendar days, in compliance with PIPA.
- If access is denied in whole or in part, the individual will receive a written explanation and information on how to appeal to the Office of the Information and Privacy Commissioner for BC.
5. Documentation:
- All requests and responses will be logged and stored securely.
- The clinic will keep a record of the reasons for any denial or redaction.
